Ask the person "where do sleep most nights?"

Use prevention ("at risk") intake form For people that are couch surfing, staying house-to-house, or staying in a room temporarily.

Use homeless individual or family assessment intake form: if the person is staying in a shelter, on the streets, in a car, or another place not meant for human habitation.


INTAKE APPOINTMENTS (HOW-TO)
  • For a Family intake, use a Household with Children form.
  • If it's a couple, fill out two single adult forms (and we can note that they are a 'family unit', but without children).
  • Under "other" in the packet - ask if they are a convicted felon. Notate their answer of being a felon, therein.


During intakes, call Owen or Rob to supervise the day shelter during that time period.

Afternoon introduction:

  • Thanks for making the time to come in. (Introduction of self)

  • I’ll be asking questions that relate to and help to determine eligibility for different programs. 

  • In this meeting, I won’t be determining whether or not you can receive any direct assistance, but you will be entering the large pool of people needing housing assistance. 

  • The programs we have do not offer immediate help, so it’s a good idea to keep looking for and working on other options.

Afternoon exit:

  • Thanks again for your time. 

  • I’ll be sharing this information with the program manager.

  • The information you have shared helps us to secure additional funding by showing our local need is greater than capacity. 

  • As I mentioned, this is the first step in what can be a long process but this assessment has put you on our radar. 

  • Please let us know if something in your situation changes.

The program manager will give you a call within the next 2 days to discuss what options are available.

We will reach out should options become available.
